Survey questions allow you to collect feedback from learners within a course. You can use them for various purposes, such as gathering demographic information or understanding participant sentiment.
These survey questions can be included alongside other activity types in any course. For instance, you might place a survey question at the beginning of a course to gauge initial sentiment or confidence and another at the end.
Keep in mind that survey questions included at the end of a course will be the last “new presentation” activity learners receive, although they may be delivered before all repeats have been given.
How does a survey question look in the learner experience?
Survey questions do not affect a course's overall proficiency. Learners receive survey questions in the same manner as other activity types and they are prompted to select either one or multiple answer choices based on the author’s settings.
- A learner will receive engagement points for answering the survey question but no “correctness” points.
- Survey questions are excluded from any proficiency calculations.
- Survey questions are not repeated, regardless of course spacing and repetition settings.
How can I create a survey question for my course?
When creating a new activity, select “Survey” from the activity type dropdown in the top right corner:

Add your desired text (and media if applicable) and answer choices.
Notice that answers are in blue, and are neither correct nor incorrect:
You have the option to specify whether learners can provide only one response or choose multiple responses using the ‘Allow Multiple Selections’ toggle:
You can also add an explanation for learners to view once they have answered the question.
How do survey questions look in reporting?
Survey results are visible to Program Managers and Reporting Managers in the "Measure" tab.
